The Homeless Outreach Program (HOP) provides services to citizens of Baltimore County who are homeless and are victims of domestic violence, have substance use, are veterans, have HIV/AIDS, have a psychiatric disaility or who are chronically homeless. We reach out to them in the shelter and on the streets. The Shelter Team offers case management services to persons in Baltimore County's shelters who are diagnosed with a mental illness. The Street Team also accepts referrals from County shelters and provides outreach to homeless individuals living in campsites and on the streets. HOP provides intensive case management by linking people to medical care, housing, entitlements and other critical resources and services
Through the Shelter Plus Care program, individuals with a psychiatric disability may obtain subsidized housing in one, two or three bedroom units. Prologue assists with all aspects of this program to support its mission of providing affordable housing and eliminating homelessness one person at a time.
