The Homeless Outreach Program (HOP) provides services to citizens of Baltimore County who are chronically homeless and need case management services. We reach out to them in the shelter and detention centers and on the streets. The Shelter/Jail Team, with offices in Towson, offers case management services to persons in Baltimore County’s detention centers who are diagnosed with a mental illness and who will be homeless when released. This team also serves mentally ill persons residing in domestic violence and transitional shelters. The Street Team, located in Dundalk, accepts referrals from freezing weather shelters and provides outreach to homeless individuals living in campsites and on the streets. HOP provides intensive case management by linking people to medical care, housing, entitlements and other critical resources.
